For New Patients

If you have a patient you wish to refer to Nashville Fertility Center, please send a referral to our medical records team at medicalrecords@nashvillefertility.com or fax to 615-320-0240. One of our new patient coordinators will contact the patient with the next steps to make an appointment. Alternatively, ask the patient to visit our website, click on New Patients, and follow the instructions. A Medical Records Release Form should be completed by the patient to have pertinent records from your office sent to Nashville Fertility Center before their appointment.

To Order a Semen Analysis

For doctors who wish to refer their male patients for semen analysis, please print and fill out our Ovation Andrology Lab Request, and fax to 615-320-0240 or email to medicalrecords@nashvillefertility.com. Please also give the original to your patient. This ensures that you receive your patient’s results in a timely manner.
